United Hospice of the Piedmont - Columbia, part ! of the UHS-Pruitt Corporation family of companies, has an opportunity for a Full Time RN Case Manager in the Columbia, SC area.
SUMMARY OF DUTIES: Assumes responsibility and accountability for the provision of patient care according to professional standards of nursing practice in compliance with state and federal laws and regulations. Implements nursing policies and procedures in accordance with the hospice philosophy and all state and federal regulations. Conducts screenings, admissions, education and on-call functions as assigned. Utilizes resources in an efficient and economic manner to ensure each patient achieves the highest practicable physical, mental, and social well-being. Performs any and all professional nursing duties as determined by qualifications and training.
QUALIFICATIONS: Qualified candidates will be an RN with CPR certification. Previous hospice and or home health experience preferred.
EDUCATION: Registered Nurse with cu! rrent active state license and a graduate of an accredited sch! ool of nursing.
EXPERIENCE: 1 year of professional experience in a hospice or homecare setting. 3 years of professional experience in a hospice or homecare setting is preferred.
